A 38yr old male resident of chityala,bore well worker by occupation came to our hospital with complaints of stomach pain and vomiting since 3 days ... HISTORY OF PRESENT ILLNESS
Patient was apparently asymptomatic 1years back
As per the patient he suffered from episodes of stomach pain occasionally but it does not affect his daily route 6 months back he suffered with severe
stomach pain and consulted doctor in nalgonda and given a medication for 1 month and three days bach he suffered with stomach pain and shoulder pain and vomitings and adimitted in hospital ,he is suffering from left shoulder pain since 3 days
Past history
No history of hypertension diabetes epilepsy and tuberculosis
PERSONAL HISTORY
He has mixed duet
Bowel and bladder movements-normal
Adequate sleep since 3 days with inadequate sleep due to pain, patient was alcoholic since 8 years,he has history of chewing pan 2 years back
FAMILY HISTORY
No relevant family history
GENERAL EXAMINATION
Patient was conscious,coherent and cooperative
Moderately built
No pallor
No cyanosis
Clubbing-present
No generalized lymphadenopathy
VITALS :
Temperature-98.6°F
Respiratory rate-18cycles/min
Pulse rate-75beats/min
BP-120/80mm Hg
SYSTEMIC EXAMINATION
CVS :
S1 S2 heard
RESPIRATORY SYSTEM :
Bae present,no abnormalities
CNS :
No focal neurological deficits
PROVISIONAL DIAGNOSIS :acute pancreatitis
